Individual Health Insurance
MHealth Insured Elect is a suite of health benefit plans designed for individuals and families. The three categories of Elect plans vary in deductibles, premiums and coverage.
Elect and Elect Basic Plans
These MHealth Insured plans offer individuals and families options that include substantial health care benefits without additional cost or benefits they may not need. Our Elect and Elect Basic plans offer individuals the security and peace of mind that comes with knowing they have coverage at a price they can afford.
Our three Elect plans vary in annual deductible amounts, and each includes access to emergency care, routine health care services, and preventive care and wellness programs.
Elect Plus Plans
MHealth Insured plans offer individuals and families health benefit options with increased flexibility, more coverage and enhanced benefits. These plans feature ýfirst-dollar benefitsý (coverage with no annual deductible amount) for in-network office visits with a $30 copay, and certain preventive care screenings with a first-dollar benefit maximum of $500 per member. Additionally, Elect Plus plans include:

Coverage for outpatient medical emergency room treatment, including facility/hospital charges and physician fees that are covered at 100 percent. (subject to a $100 copay for emergencies.)

Five annual deductible options ($1,000, $1,500, $2,500, $3,500 and $5,000).

A fourth-quarter carry-over for the annual deductible.
Elect HSA-Compatible Plans
These MHealth Insured plans offer individuals and families a variety of consumer-driven health plans. An HSA is a Health Savings Account established exclusively for members to use to pay for current and future qualified medical expenses. For individuals and families to qualify for an HSA, they must be enrolled in a High-Deductible Health Plan (HDHP). UniCareýs HDHPs are HSA-Compatible, which means they are designed to meet certain requirements for annual deductibles and annual out-of-pocket expense maximums.
The HDHPs are provided by UniCare, and the HSA is administered by a bank or financial institution that is qualified to provide this service.
